Community Based Skilled Nursing (CBSN) Services

Duties for RNs and LPNs at Florence Precious Home Care include:

• Evaluation Tasks: Perform vital signs checks and review medications to ensure patients are on the right track.
• Medication Administration: Administer medications as prescribed by the patient’s physician, ensuring proper dosages and timing.
• Wound Care: Dress and undress open wounds, assess the progress of healing, and adjust care plans as needed.
• Patient and Family Education: Educate patients and their families on proper home health care strategies and procedures to ensure safe and effective care at home.
• Care Coordination: Coordinate with Occupational Therapists, Physical Therapists, Physicians, and other healthcare professionals involved in the patient’s care plan.
• Palliative Care: Provide palliative care as needed to ensure patients remain comfortable and their quality of life is maintained.
• Recommendations for Tools: Suggest devices or tools that might improve the patient’s quality of life, such as mobility aids or home modifications.
• Patient Communication: Listen to patients and respond to their concerns or requests promptly and compassionately.
• Physician Communication: Contact and take orders from the physician over the telephone for any updates or changes in the patient’s care plan.
• Invasive Procedures: Perform invasive procedures such as inserting Foley catheters, providing enemas, gastric or colostomy irrigations, suctioning, tube feeding, and inserting rectal tubes or flatus bags.
• Medication Dispensation: Dispense and administer various medications, including eye drops, ointments, skin patches, liquid medicines, ear drops, pain medications, and suppositories.
• Bowel Program: Implement and manage bowel programs to ensure patient comfort and health.
• Tracheostomy Care: Provide care for patients with tracheostomies, ensuring cleanliness and functionality.
• Application of Warm Compresses: Apply warm compresses or heating pads as needed for patient comfort.
• Urine Tests: Perform urine tests to monitor health conditions and adjust care plans accordingly.
• Glucometer Assistance: Use or assist the client with a glucometer to monitor blood glucose levels.
• Oxygen Flow Regulation: Regulate the flow rate of oxygen for patients requiring supplemental oxygen.
• Heavy House Cleaning: Perform heavy house cleaning tasks such as washing walls or windows, moving heavy furniture, and cleaning basements.
• Service Recommendations: Additional services or interventions are recommended to address and treat patient problems.
• Nail Care: Provide basic nail care, avoiding tasks like cutting and trimming toenails or fingernails to prevent injury.
